First off, for FIX 4.3 and above, FPL doesn't assign exchange codes at all; these are ISO MIC codes, which are the ISO 10383 standard. The list of MIC codes, as well as forms for requesting new MIC codes if they don't exist, are available at this URL:

For FIX 4.2 and below, FIX uses Reuters exchange codes, and we assign numeric codes for any exchanges that don't have Reuters codes assigned. These are listed here:

There is a second tab for the numeric FPL-assigned exchange codes.
Requests for numeric exchange codes can be made to fpl@fixprotocol.org, however we request that someone from the exchange itself makes the request.
